Connelly-McKinley Funeral Homes provides the families who entrust their loved one’s funeral arrangements to our care with a comprehensive kit that contains important documents required to apply for Canada Pension Plan Benefits, contact information and instructions on various estate matters.
This After Care kit will be given to you by your Funeral Director at the time of services or, if you are meeting with a Family Service Consultant, during your scheduled appointment.

Completed applications for Death Benefits may be submitted either through the mail or in person by making an appointment.

When making your inquiry, you may be required to provide the following information/documents:
Funeral Directors Statement of Death (obtained from Connelly-McKinley Funeral Homes)
Deceased’s birth certificate
Birth certificate of person(s) making the application
Marriage certificate (if applicable)
Proof that the deceased contributed to the Canada Pension Plan (e.g., pay stubs; T4 slips)
