Connelly-McKinley Funeral Homes provides to the families who entrust in our care - their loved ones funeral arrangements, with a package that contains important documents required when applying for Canada Pension Benefits. This package is usually located in the Connelly-McKinley portfolio case you received, the day of the funeral service.

For those family’s meeting with a Family Care counselor, the package will be presented to you during your scheduled appointment.

Completed applications for Death Benefits may be submitted either through the mail or in person by making an appointment.

When making inquires into Canada Pension benefits, you may be required to provide the following information / documents:

  • Funeral Directors Statement of Death (obtained from Connelly-McKinley Funeral Homes)
  • Deceased’s Birth Certificate
  • Birth Certificate of person(s) making application
  • Marriage Certificate (if applicable)
  • Proof that the deceased’s paid into Canada Pension

Examples: pay-stubs, past T4 tax slips
